Referrals Terms and Conditions
We are a registered charity based in Leeds that provides vulnerable families with children aged 0-5 years with baby food, supplies, clothes, and equipment in times of crisis.
1. Referrals
Referrals can only be made by a care professional or agency with a valid work email. This registration process will lead to an approval of the professional/agency, once checks have been made.
We support families with children aged 0-5 years, who have a Leeds postcode
2. Our process for registering new referral partners
Potential referral partners register with this form
Due diligence checks will take place, and the referral partner is approved in our system.
We will send a link to a portal where our referral partners log in and refer a family
We will contact the family to establish what items they require. We will do this as soon as possible, but timescales vary depending on our waiting list. If the referral is an emergency, please mark it as urgent. Please note, if we do not hear back from the referral partner or service user after three attempts we reserve the right to cancel the referral.
When items are ready to collect the referral partner will be sent a booking link
When the item is ready for collection, we will arrange a collection with you via our booking system. Alternatively, our delivery driver will take the items directly to the family’s address.
We have limited collection opening hours: Monday 8:30am - 14:00pm, Wednesday 10:00 - 13:00, Thursday 8:30am - 14:00pm and Friday 8.30am - 13.00pm. All collections need to be booked in with us at least 48 hours in advance, as we are only able to accommodate 1 vehicle at the same time.

I will explain to the service users I am referring, that most of the items supplied have been donated to Leeds Baby Bank by the public and accepted in good faith. Leeds Baby Bank does not accept any responsibility for the items and cannot guarantee the reliability and safety of any of the items. We strongly recommend that they check and clean the items thoroughly before use.

I confirm that I will ask consent of my service users for Leeds Baby Bank to contact them and consent to share the information provided in the referral portal for the purposes of accessing Leeds Baby Bank services.

Data Protection Law recognises that some categories of personal information are more sensitive. Sensitive Personal Information can include information about a person’s health, race, ethnic origin, political opinions, sex life, sexual orientation or religious beliefs.
4. GDPR
Leeds Baby Bank, inline with GDPR regulations will ensure that personal data will:
Be obtained fairly and lawfully and shall not be processed unless certain conditions are met
Be obtained for a specific and lawful purpose
Be adequate, relevant but not excessive
Be accurate and kept up to date
Not be held longer than necessary
Be processed in accordance with the rights of data subjects
Be subject to appropriate security measures
Not to be transferred outside the European Economic Area (EEA)
Data collection is required for statistical analysis, future funding and evaluation purposes.
